PhilHealth Identification Number (PIN)

A unique permanent member number issued when you register. Hospitals and clinics use it to look you up in the PhilHealth system. You only get one PIN for life — even if you change employer, marital status, or category.

Member Data Record (MDR)

A printable PDF (and the official source-of-truth document) that lists your PIN, name, date of birth, membership category, employer (if employed), qualified dependents, and contribution status. You download a fresh copy from the Member Portal whenever your details change.

PhilHealth Member Registration Form (PMRF)

The paper form (PhilHealth Form M1/M2 in current revisions) you fill out to register, update your records, or add dependents. You don't use the PMRF day-to-day — it's the on-ramp into the PhilHealth database that ultimately produces your MDR.

Find out if you already have a PIN

Under Universal Health Care, every Filipino is enrolled, but you may already have a PIN assigned from a previous employer, school, or LGU registration.

  • Employed members: check your latest payslip — the PIN is printed alongside the PhilHealth deduction. Or ask your HR.
  • Returning members: log in to the Member Portal at memberinquiry.philhealth.gov.ph using your old PIN and date of birth.
  • If you can't find it: call the Action Center at (02) 8866-225-88 or message PhilHealth on Facebook with your full name and date of birth.

If you have no PIN yet, register online via ERS

Use the PhilHealth Electronic Registration System (ERS) at eregister.philhealth.gov.ph:

  • Fill in your full name, date of birth, sex, civil status, address, and contact details

Create a Member Portal account

With your PIN in hand, go to memberinquiry.philhealth.gov.ph and click "Register". You need your PIN, an active email address, and your date of birth as registered with PhilHealth.

Download and print your MDR

Inside the Member Portal:

  • Navigate to "MDR" or"Member Data Record"

Use your PIN and MDR

When you transact with a hospital, accredited clinic, or another government office:

  • Present a valid government-issued ID and your PIN (and the printed MDR if requested)
  • Hospitals will look you up in the PhilHealth system using your PIN and verify your active membership and dependents
  • For outpatient/primary-care benefits under YAKAP, first self-assign to a YAKAP-accredited clinic via the Member Portal or the eGovPH app, then complete a First Patient Encounter (FPE) at that clinic
